· Projects providing screening services must document a mechanism to provide the necessary follow-up services if they will not provide the services themselves (e.g. written documentation outlining a referral system or partnerships in place).

· Applicant has tax exempt status under the Internal Revenue Service code.

· Applicant must be a non-profit organization located in or providing services to one or more of the following locations Acadia, Allen, Ascension, Assumption, Avoyelles, Beauregard, Bienville, Bossier, Caddo, Calcasieu, Caldwell, Cameron, Catahoula, Claiborne, Concordia, De Soto, East Baton Rouge, East Carroll, East Feliciana, Evangeline, Franklin, Grant, Iberia, Iberville, Jackson, Jefferson Davis, La Salle, Lafayette, Lafourche, Lincoln, Livingston, Madison, Morehouse, Natchitoches, Ouachita, Pointe Coupee, Rapides, Red River, Richland, Sabine, St. Helena, St. James, St. Landry, St. Martin, St. Mary, Tangipahoa, Tensas, Terrebonne, Union, Vermillion, Vernon, Webster, West Baton Rouge, West Carroll, West Feliciana, and/or Winn Parishes.
